About the Commonwealth Home Support Programme (CHSP)
The CHSP provides entry-level support for older people who need some help to stay at home. Service providers work with them to maintain their independence. Support can include help with daily tasks, home modifications, transport, social support and nursing care.
It also provides respite services to give carers a break.
The program aims to:
- help people live as independently as possible
- focus on working with them, rather than doing things for them
- give a small amount of help to a large number of people.
Most people in the CHSP only need 1 or 2 services to help them stay independent.
Reliable Community Care Pty ltd is subsidised by the Commonwealth Government to provide entry level Centre Based Day Respite care through the Commonwealth Home Support Program (CHSP). The Program is provided to people who require assistance to keep living independently in their own home and community.
We are proud to provide following services to people over 65 years of age who are from Culturally and Linguistically background.
Our current services are:
- Social Support Individual (web/telephone contact)
- Domestic Assistance (unaccompanied shopping)
- Centre Based Day Respite care is currently on hold due to COVID restriction in Victoria
Who it is for?
The CHSP is for frail older Australians who need support to live independently at home and are either aged 65 years or over (50 or over for Aboriginal or Torres Strait Islander peoples) and have functional limitations and need assistance
The CHSP also supports carers by providing planned respite services for CHSP recipients. This allows carers to take a break from their caring duties.
